What is Rhinoplasty?

A primary Rhinoplasty, or a “Nose Job”, is an intricate, multifaceted surgical procedure that requires meticulous attention to detail and the ability to assess how small changes to the patient’s nose and nasal structure will impact the patient’s facial symmetry and proportions. Even under ideal circumstances, the potential for post-rhinoplasty cosmetic and/or functional complications to develop is very real—and the risk of these complications is magnified when the procedure is performed by a plastic surgeon without specialized training in facial surgery.

Rhinoplasty Techniques

As a skilled and highly trained facial plastic surgeon, Dr. Cohn uses a variety of techniques and methods to enhance your nose. Below are some of the more common methods of how he can perform nose surgery.

Open Rhinoplasty

Open Rhinoplasty begins with a small incision across the columella, which separates the two nostrils. From here, Dr. Cohn will elevate the skin to assess your bony and cartilage framework. At this point, he will have full exposure to performing corrections such as removing excess tissue, reshaping the cartilage and nasal bones, or placing cartilage grafts. Once his corrections are finished, the skin is repositioned and sutured for healing.

An Open Rhinoplasty will result in a scar at the incision site, however, this is usually well concealed. As time passes, the scar will fade and remain minimally visible.

Closed Rhinoplasty

The Closed Rhinoplasty technique is employed for patients needing minimal nasal correction. Dr. Cohn initiates this nose surgery by making small incisions inside each nostril. Subsequently, he performs necessary nasal corrections, such as sculpting the cartilage and nasal bones, removing excess tissue, or modifying the shape and size of the nostrils. Upon completion, the incisions are closed using dissolvable sutures.

Closed Rhinoplasty leaves no visible scarring, but due to limited access to this technique, not all patients are potential candidates.

Revision Rhinoplasty

When conducted by a skilled plastic surgeon, Rhinoplasty surgery can yield fantastic results. Unfortunately, some Rhinoplasty patients with prior nasal surgeries may not achieve the desired outcomes. Revision Rhinoplasty, also known as Secondary Rhinoplasty, is often deemed a more intricate cosmetic procedure by many plastic surgeons, given the heightened expectations of Rhinoplasty patients. Consequently, numerous Rhinoplasty surgeons may struggle to meet the specific results sought by their patients.

Male Rhinoplasty

Non-Surgical Rhinoplasty

Rhinoplasty is a surgical procedure, making the term Non-Surgical Rhinoplasty seem somewhat paradoxical, yet, this minimally invasive alternative (also known as Liquid Rhinoplasty), can be highly effective for patients dealing with minor contour deformities. By using fillers like those used for augmenting other areas of your face, issues like dorsal depressions or flat nasal tips can be addressed, smoothing the contour or enhancing tip projection.

Although the effects of fillers are temporary, they offer a great option for individuals not yet ready for surgery or seeking a quick solution for upcoming events such as weddings, graduations, or reunions.

Non-surgical rhinoplasty is performed in an office setting and typically takes about 45 minutes. If you’re considering this option for an imminent event, it’s advisable to allow a few days for the filler to settle in. Dr. Cohn can guide you in determining if this is the appropriate treatment for your needs.

Who are the best candidates for the Rhinoplasty procedure?

If you are interested in getting a Rhinoplasty in Long Island, the first thing you will need to do is schedule a consultation with Dr. Cohn. We always encourage our patients to do at least a little bit of research prior to their appointment so that they can be well prepared before this appointment. Keep in mind that it’s completely fine to come in with a basic understanding, as we have no problem answering any questions you may have, however, we believe that the more knowledgeable you are, the better off you will be.

When you come to meet with Dr. Cohn for your Rhinoplasty consultation, he will go over your medical history, talk about your desired look, and evaluate your nose and face. You will even be invited to look at our rhinoplasty photo gallery to see the changes that can be made. Once he has a complete understanding of what will work best for you, he will create your surgical plan.

If the two of you agree with this plan, our Patient Coordinator will be able to help you schedule a surgical date. They will also provide you with some instructions to follow to ensure a successful surgery. This may include getting off of blood-thinning medications, organizing a friend or caretaker after the procedure, and getting the proper medications to take after your surgery

What to expect on the day of Rhinoplasty surgery

On the day of your nasal surgery, you are placed under general anesthesia or local anesthesia, depending on the extent of your procedure and what is discussed during your consultation.

Dr. Cohn will most likely use one of two techniques when performing your nose surgery: Open Rhinoplasty or Closed Rhinoplasty.

If you are looking for minor cosmetic changes, a closed procedure is frequently used. With this technique, the incisions are created on the inside of the nose. Dr. Cohn uses small surgical tools to manipulate your nasal tissue to produce the desired result.

The open technique involves incisions in the columella, the strip of skin between the nostrils; the top layers of skin are then lifted back. This procedure allows greater visibility and manipulation of the underlying nasal structures. The results are also more predictable.

The surgery takes anywhere from one to three hours depending on the complexity of the operation. During your consultation, Dr. Cohn will be able to give you a more realistic expectation of how long it will take.

Rhinoplasty recovery

After you wake up from your rhinoplasty surgery, your nose will have an external splint and a mustache dressing. Your face will most likely feel sore. You will have some splints inside of your nose, which will keep the tissues intact during the healing process. You will be given a prescription for pain medication some antibiotics and post-operative instructions on how to care for yourself. The bandages are usually removed seven days after surgery.

During the days and weeks following nose surgery, you should keep your head elevated when sleeping and avoid smoking, blowing your nose, and participating in strenuous physical activities. Our team will closely monitor your healing process, with the expectation that you return the following day or two so that any bandages can be redressed and Dr. Cohn can observe your initial recovery.

You will most likely be able to return to your daily routine within 10 days. Please note that while your nose will heal fully within a few weeks, the results will become visible gradually, and the outcome may not be evident for up to 12 months.
