What is Gender Affirming Surgery?

Gender Affirming Surgery is typically a series of procedures designed to help patients have a facial type that aligns closer to their gender identity. Gender affirmation surgery may be an option if your sex assigned at birth differs from your gender identity and may help if you’ve experienced any psychological distress because of gender dysphoria.

Dr. Cohn and his team provide a wide-ranging set of options for masculinizing/feminizing procedures to help alter your face’s physical appearance to reflect your gender identity. 

Some more common facial procedures performed during Gender Affirming Surgery include Rhinoplasty, Brow Lift, Jaw Contouring, Chin Surgery, and Facelift. Below, we have included some brief descriptions of how these treatments can provide you with a face you choose to identify with.

Facial Masculinization Surgery (FMS) – Female to Male

Chin Implant

Dr. Cohn can use an implant or Facial Fat Transfer to create a more prominent and sharper jawline. 

Brow Lift

This surgery can create a lower and wider forehead. It may flatten the ridge where your eyebrows sit and can lengthen the distance between the eyebrows and the hairline.

Jawline Contouring

Wider angles and more prominent chins tend to confer a more masculine appearance, so Dr. Cohn can increase the size of your jaw to achieve a more robust and square jaw with a sharper mandibular angle.

Rhinoplasty

Using cartilage or bone grafting, Dr. Cohn can make a larger and more projected nose that is more commonly associated with men.

Facial Feminization Surgery (FFS) – Male to Female

Brow Lift/ Eyelid Lift

Dr. Cohn can elevate your brow to give your eyes a more feminine appearance. He will make four or five tiny incisions (about 1cm in length) in either the hairline or brow line. Afterward, he will pull your skin higher to lift your eyebrows above the orbital rim.

Rhinoplasty

Dr. Cohn can reshape the underlying bone and cartilage in your nose to reduce its width and shape. A rhinoplasty can also change the shape of the bridge and tip of your nose to create a more delicate appearance.

Lip lift and augmentation

This procedure can raise the height of your top lip with a small incision made under your nostrils for a long-lasting result. If you’re interested in something a little more temporary, Dr. Cohn can make your lips fuller with a Facial Fat Transfer or Dermal Fillers.

Jawline Contouring

Through an incision inside your mouth, Dr. Cohn can reshape the bone by shaving it down or trimming it in order to reduce the size and angle of your jaw, creating a softer angle between the ear and the chin.

During this appointment, you will be able to ask any questions you may have, discuss your facial aesthetic goals, and learn about all of your options. Dr. Cohn will ask you about your medical and lifestyle history to determine if this is the safest and best procedure for you. It’s very important to be completely honest with Dr. Cohn during this session as it will help prevent any unnecessary risks. 

If Gender Affirming Surgery is deemed appropriate for you, then Dr. Cohn will create your customized surgical plan and our Patient Coordinator will help you schedule your surgical date. We will also provide you with some instructions to follow that will help you have a successful and safe procedure.

What to expect on the day of Gender Affirming Surgery

On the day of your Gender Affirming Surgery in Long Island, once you arrive at the surgical facility, Dr. Cohn will meet with you and will walk you through what will happen during surgery. You will then meet with our Anesthesiologist who will sedate you with general anesthesia or IV sedation for a more comfortable experience.

Depending on the surgery, Dr. Cohn will perform the necessary steps to complete your FMS or FSS. After the surgery is finished, we will monitor you in our recovery room until you awake from sedation.

Gender Affirming Surgery recovery

You should be able to return home after having most of the procedures, however, there is a possibility that we may keep you overnight to continue to monitor your initial healing. We will provide you with some post-op instructions to follow which may include how to care for your surgical site(s) after surgery, which medications to apply or take orally to aid healing and reduce the risk of infection, specific concerns to look for, and when you are expected to follow up with Dr. Cohn.

Your recovery times vary based on what procedures or combination of procedures you have, and every patient will have their own recovery experience.

You should expect to avoid any strenuous or rigorous activities for at least a month. During your follow-up visits, Dr. Cohn and his team will keep you informed of when it’s ok to resume certain activities.
